Unfortunately, I've come across this many times amongst civil servants, ans public servants in general - they're just too lazy and incompetent to do what they're paid to do.

This is what they admit to! My bet is the true figure is several times higher than this. Theresa May (and the Home Office) is not fit for purpose.
